Ivermectin tablets: strengths, brands, uses and prices

What ivermectin is and how it works
Ivermectin is an antiparasitic medicine from the avermectin family. It binds to a type of chloride channel found in the nerve and muscle cells of worms and mites. That paralyses the parasite, which then dies or is cleared by the body. At normal doses, human nerve cells are largely protected because the blood–brain barrier keeps the drug out, which is why ivermectin has a long safety record when used as prescribed.
The discovery of avermectins earned William Campbell and Satoshi Ōmura a share of the 2015 Nobel Prize in Physiology or Medicine, and ivermectin is on the World Health Organization’s list of essential medicines. What ivermectin is used to treat
Ivermectin is used for specific parasitic infections. A doctor decides whether it’s the right medicine based on a diagnosis, often confirmed with a stool test or skin scraping.
Ivermectin tablets
- Intestinal strongyloidiasis, an infection with the threadworm Strongyloides stercoralis that can persist for decades. Read our strongyloidiasis treatment guide.
- Onchocerciasis (river blindness), a parasitic infection spread by blackflies in parts of Africa and Latin America.
- Scabies, especially crusted scabies or outbreaks where creams such as permethrin haven’t worked. See our ivermectin for scabies guide.
- Some other worm infections, as decided by a doctor. For pinworm, roundworm and hookworm, mebendazole or albendazole are usually first choice.
Ivermectin cream and lotion

Large randomised trials found that ivermectin did not help people recover from COVID-19, and it is not recommended for treating it. It is also not an approved cancer treatment. Never use veterinary ivermectin (pastes, pour-ons or injectables made for animals): these are made at different concentrations and can cause serious poisoning in people.

Ivermectin strengths: 3 mg vs 6 mg vs 12 mg
Oral ivermectin is dosed by body weight, in micrograms per kilogram. Prescribing information for strongyloidiasis uses about 200 micrograms per kilogram as a single dose, and onchocerciasis about 150 micrograms per kilogram. Because people weigh different amounts, tablets come in several strengths so the prescribed dose can be made up with as few tablets as possible.
As a simple example of the arithmetic, 200 micrograms per kilogram for a 60 kg adult works out at 12 mg: one 12 mg tablet, two 6 mg tablets or four 3 mg tablets. The total dose is identical; only the number of tablets changes. Your prescriber calculates your dose, so always follow the label rather than this example. You may also see higher-strength tablets (such as 20 mg or 24 mg) sold internationally. These only make sense when a prescriber has calculated a higher total dose. Taking more ivermectin than prescribed doesn’t make it work better and raises the risk of side effects such as dizziness, confusion and loss of balance.

How to take ivermectin tablets
- Take exactly what was prescribed. Most courses are a single dose. Some, such as scabies treatment, use a second dose 7 to 14 days later because eggs that survived the first dose hatch in that window.
- Take it on an empty stomach with water, as the prescribing information advises, unless your doctor tells you otherwise.
- Don’t split doses across days or adjust the dose yourself. If a tablet is scored, ask your pharmacist before splitting it.
- Expect follow-up. For strongyloidiasis, doctors usually repeat stool tests to confirm the infection has cleared.
- Itching can continue after scabies treatment for two to four weeks even when the mites are gone. That’s not necessarily treatment failure; check with your doctor if it persists.
- Keep it to yourself. Don’t share tablets: the right dose depends on weight and diagnosis.

Side effects, interactions and who should be careful
Most people take ivermectin without problems. Commonly reported side effects include dizziness, nausea, diarrhoea, tiredness, itching and rash. When ivermectin treats onchocerciasis, the dying parasites can trigger a reaction (fever, itching, joint pain, swollen glands) known as the Mazzotti reaction.
- Have lived in parts of Central or West Africa where the eye worm Loa loa is common. Heavy Loa loa infection can cause serious brain reactions after ivermectin, so testing is needed first.
- Take blood thinners such as warfarin. Changes in INR have been reported.
- Are pregnant, planning pregnancy or breastfeeding.
- Are treating a child who weighs less than 15 kg. Safety of tablets hasn’t been established at this weight.
- Have liver disease or a weakened immune system.
