Type 2 diabetes in brief
In type 2 diabetes, the body doesn’t respond well to insulin and, over time, the pancreas can’t make enough to keep blood sugar in range. High blood sugar over years damages blood vessels and nerves, which raises the risk of heart disease, stroke, kidney disease, eye damage and foot problems.
Treatment aims to keep your HbA1c (a measure of average blood sugar over about three months) at a target your doctor sets, often around 7% (53 mmol/mol) for many adults, while also protecting the heart and kidneys. That’s why diabetes care usually includes blood pressure and cholesterol treatment too. See our blood pressure medication guide and list of cholesterol medications.
The main types of diabetes medicine
| Class | How it works | Examples | Good to know |
|---|---|---|---|
| Biguanides | Reduces sugar released by the liver | Metformin | Usually the first medicine; stomach upset is common at first |
| GLP-1 receptor agonists | Boost insulin when sugar is high, slow stomach emptying, reduce appetite | Semaglutide, Liraglutide, Dulaglutide | Oral semaglutide is the only tablet; others are injections |
| SGLT2 inhibitors | Help the kidneys pass excess sugar in urine | Empagliflozin, Dapagliflozin, Canagliflozin | Also protect the heart and kidneys |
| DPP-4 inhibitors | Gently increase the body’s own GLP-1 | Sitagliptin, Linagliptin, Saxagliptin, Alogliptin | Weight-neutral, well tolerated |
| Sulfonylureas | Make the pancreas release more insulin | Glipizide, Glimepiride, Glyburide | Inexpensive; can cause low blood sugar |
| Thiazolidinediones | Improve the body’s response to insulin | Pioglitazone | Can cause fluid retention |
| Insulin | Replaces or adds to the body’s own insulin | Glargine, Degludec, Lispro, Aspart | Needs careful dosing and glucose monitoring |

Oral semaglutide (Rybelsus): strengths and how it's started
Rybelsus is semaglutide in tablet form, approved for adults with type 2 diabetes. It’s the same medicine family as injectable semaglutide, but formulated with an absorption enhancer so it can survive the stomach. The dose is built up in steps to reduce stomach side effects:
| Strength | Role | Product |
|---|---|---|
| 3 mg | Starting dose for the first 30 days, to get used to the medicine; not intended to control blood sugar on its own | Semaglutide 3 mg (Rybelsus) |
| 7 mg | The usual maintenance dose after the first month | Semaglutide 7 mg (Rybelsus) |
| 14 mg | A higher dose if more control is needed after at least 30 days on 7 mg | Semaglutide 14 mg (Rybelsus) |
Our detailed article Rybelsus 3 mg vs 7 mg vs 14 mg covers each step, and oral semaglutide tablets: uses, safety and how they compare with injections explains the differences from injectable versions. Weight-management versions of semaglutide are separate products at different doses; Rybelsus 3, 7 and 14 mg are diabetes medicines.
How to take Rybelsus correctly
Oral semaglutide is absorbed only in a narrow window, so the routine really matters. Taking it with food or too much water can make it barely work.
- Take it first thing in the morning on an empty stomach.
- Swallow the tablet whole with no more than 120 mL (half a cup) of plain water.
- Wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.
- Keep tablets in their original blister until you take them.
- If you miss a dose, skip it and take the next one the following morning.
If you also take levothyroxine, which needs a similar empty-stomach routine, ask your doctor how to time the two.
Rybelsus side effects and safety
The most common side effects are nausea, diarrhoea, vomiting, stomach pain and reduced appetite, mostly in the first weeks and after dose increases. Eating smaller meals and stopping when full helps.
- Don’t take semaglutide if you or a close family member has had medullary thyroid cancer, or if you have multiple endocrine neoplasia type 2 (MEN 2).
- Get urgent help for severe, persistent stomach pain that may spread to the back, which can be a sign of pancreatitis.
- Tell your doctor about any history of pancreatitis, diabetic eye disease, kidney problems or gallbladder disease.
- Combined with insulin or a sulfonylurea such as glipizide, semaglutide raises the risk of low blood sugar; doses may need adjusting.
- Vomiting and diarrhoea can dehydrate you and affect the kidneys, so keep drinking fluids.
- It isn’t recommended in pregnancy; stop and talk to your doctor if you’re planning to conceive.
GLP-1 medicines are heavily counterfeited. Read 7 warning signs of fake GLP-1 drugs online before buying from anywhere.
Glipizide and low blood sugar
Glipizide is a sulfonylurea: an inexpensive, effective medicine that prompts the pancreas to release more insulin. See Glipizide 2.5 mg (Glez). It’s usually taken about 30 minutes before a meal. Because it works regardless of your blood sugar level, it can cause hypoglycaemia, especially if you skip meals, drink alcohol, exercise more than usual or take it with other diabetes medicines.
Recognising and treating low blood sugar
- Signs: shakiness, sweating, hunger, fast heartbeat, confusion, irritability, blurred vision.
- Treat it fast: take 15–20 g of fast-acting sugar (glucose tablets, juice or regular soft drink), recheck after 15 minutes, then eat a snack.
- Get emergency help if someone is confused, drowsy or can’t swallow safely.
Weight management alongside diabetes
Losing even 5–10% of body weight can improve blood sugar, blood pressure and cholesterol. Diet and activity come first, and some diabetes medicines (GLP-1s and SGLT2 inhibitors) also help with weight.
Orlistat is a different kind of weight-loss medicine that blocks some of the fat you eat from being absorbed. See Orlistat 60 mg (Slim Trim) and Orlistat 120 mg (Slim Trim), or browse weight loss. It works best with a reduced-fat diet; eating high-fat meals with it causes oily, urgent bowel movements. Because it reduces absorption of some vitamins, a multivitamin taken at bedtime is usually recommended. Check with your doctor if you take levothyroxine or other daily medicines, as timing may need adjusting.
Day-to-day diabetes management
- Know your targets: HbA1c, blood pressure and cholesterol targets set by your doctor.
- Annual checks: eye screening, kidney blood and urine tests, and foot checks.
- Sick-day rules: during vomiting, diarrhoea or fever, some medicines (including metformin and SGLT2 inhibitors) may need pausing. Ask your doctor for written sick-day rules.
- Never run out: plan refills ahead of delivery times. See how to refill a prescription.

Rybelsus, glipizide and everyday management.
Still have a question? Contact our team.
Is Rybelsus the same as Ozempic?
Both contain semaglutide. Rybelsus is a daily tablet; Ozempic is a weekly injection. Doses aren’t interchangeable, so switching needs a doctor’s plan.
Can I take Rybelsus with coffee?
No. Take it with up to 120 mL of plain water only, then wait at least 30 minutes before coffee, food or other medicines.
Why do I start on 3 mg?
The 3 mg dose for the first 30 days helps your stomach adjust. It isn’t meant to control blood sugar by itself; most people then move to 7 mg.
Is Rybelsus approved for weight loss?
Rybelsus 3, 7 and 14 mg are approved for type 2 diabetes. Weight loss can be a side benefit, but weight-management semaglutide products are separate medicines.
Does glipizide cause low blood sugar?
It can, especially with missed meals, alcohol or other diabetes medicines. Learn the signs and what to do.
Can I take metformin with Rybelsus?
Yes, they’re often used together. Your doctor will decide the combination and doses.
How should I store Rybelsus?
Keep tablets in the original blister pack, away from moisture, until you take them.
