Oral ivermectin is dosed by body weight. The FDA-approved label for Stromectol (ivermectin 3 mg tablets) uses a single dose of about 200 micrograms per kilogram (mcg/kg) for intestinal strongyloidiasis and about 150 mcg/kg for onchocerciasis (river blindness), with weight bands that round to whole tablets. For scabies, the CDC lists oral ivermectin at 200 mcg/kg as a treatment option, given twice 7–14 days apart, with a longer schedule for crusted scabies.
When you press Calculate, the tool:
| Body weight | Strongyloidiasis (≈200 mcg/kg) | Body weight | Onchocerciasis (≈150 mcg/kg) |
|---|---|---|---|
| 15–24 kg | 3 mg (1 × 3 mg) | 15–25 kg | 3 mg (1 × 3 mg) |
| 25–35 kg | 6 mg (2 × 3 mg) | 26–44 kg | 6 mg (2 × 3 mg) |
| 36–50 kg | 9 mg (3 × 3 mg) | 45–64 kg | 9 mg (3 × 3 mg) |
| 51–65 kg | 12 mg (4 × 3 mg) | 65–84 kg | 12 mg (4 × 3 mg) |
| 66–79 kg | 15 mg (5 × 3 mg) | 85 kg and over | 150 mcg/kg |
| 80 kg and over | 200 mcg/kg | – | – |
| Type | Dose | Schedule |
|---|---|---|
| Classic scabies | 200 mcg/kg | Day 1, then repeat 7–14 days later (the second dose kills newly hatched mites) |
| Crusted scabies | 200 mcg/kg | Days 1, 2, 8, 9 and 15 (plus 22 and 29 if severe), with topical permethrin 5% or benzyl benzoate, under specialist care |
Most people have no or mild side effects such as dizziness, nausea, diarrhoea, itching or tiredness. In onchocerciasis, itching, rash, swollen glands, fever and joint pain can occur as the parasites die (the Mazzotti reaction). Seek urgent medical help for confusion, severe dizziness, unsteadiness, seizures, a severe skin rash or difficulty breathing. Taking more than prescribed, or using animal products, has caused overdoses with these symptoms.
This tool does not calculate doses for COVID-19, cancer or other unapproved uses. The FDA has not approved ivermectin for COVID-19, and large randomized trials found no clinically meaningful benefit. Veterinary ivermectin products are made for animals and must never be used by people.

The FDA label uses about 200 mcg/kg for strongyloidiasis and about 150 mcg/kg for onchocerciasis, rounded into whole 3 mg tablets using a weight-band table. For a 70 kg adult with strongyloidiasis that is 15 mg, or five 3 mg tablets.
For strongyloidiasis, the label weight band for 51–65 kg is 12 mg (four 3 mg tablets). For scabies at 200 mcg/kg, it is also about 12 mg. Your prescriber confirms the dose.
Divide the dose in mg by 3. The calculator also shows equivalent combinations of 6 mg and 12 mg tablets.
The Stromectol label directs taking it on an empty stomach with water. Food increases absorption, which is why some guidance differs, so follow your prescriber’s instructions.
For classic scabies the CDC lists 200 mcg/kg repeated after 7–14 days, because ivermectin does not kill eggs. Crusted scabies needs more doses plus a topical treatment under specialist care.
Its half-life is about 18 hours, according to the label, so most of a single dose is cleared within a few days.
Safety has not been established in children under 15 kg or in pregnancy. A doctor should choose the treatment in these cases.
Raised INR has been reported with warfarin. Check your full medicine list with the MyRxBox drug interaction checker and your pharmacist.
